Before a new employee joins your workforce, we advise a paper screen , simple health questionnaire, to enable the employee to inform us if there are any medical problems which may affect his/her ability to perform some or all of his/her job role. If there are concerns raised by the employee, we offer a telephone or face to face assessment to advise you of any particular problems they may encounter in their new role and reasonable adjustments you can make to support them. If necessary we can offer a consultation with an Occupational Health Physician. We also give individual guidance on the Equality Act (2010).
We recommend that the medical questionnaire is sent to us directly and that all medical information is kept in confidence. Only a functional report will be sent to the employer with the employee’s consent.
